Hero Award Nomination Forms
The Safety Council of Northwest Ohio is pleased to announce that we are accepting applications for anyone in the Northwest Ohio and Lower Michigan area that has gone “above and beyond” the call of duty in saving someone’s life. There are three different categories:
? The “Hero Award” goes to persons who have risked their own lives or well being to rescue or save the life of another.
? The “Good Samaritan Award” goes to persons who have saved a life or have acted in behalf of their fellow person by out-of-the-ordinary assistance at no risk to their own lives.
? The “Certificate of Appreciation” goes to Law Enforcement Officers, Fire Fighters, Paramedics, Lifeguards, etc. (Professionals, while on duty) for their acts above and beyond the call of duty, on behalf of others.
Attached is the form for the 2008 year to recognize that person or visit our Website www.scnwo.com . The nomination forms are due by January 18, 2009 for consideration.
